ATF Form 4 Disapproval Checklist

Thanks to u/Smart_Slice_140 on Reddit, we have a copy of the ATF's Form 4 Disapproval Checklist that is used by the ATF to approve or deny applications.  This is very helpful information and would be good to use as a checklist before applying for a ATF Form 4 tax stamp application.

The Form 4 (Individual) is disapproved for the following reasons, only:

1. Fingerprint Card (FPC) never received. (As per instructions on form)
2. Photo never received.
3. Physical Address is missing.
4. Serial Number is Incorrect
5. Not registered to Transferor
   
6. CLEO is blank, or CLEO is in different State from the applicant.
7. An Unlawful reason provided for reason obtaining Firearm (Section 13)
8. Prohibiting question(s) answered, "Yes" or Blank/Missing Questions (Paper)
9. State Law Violation
10. No response to request for corrections (if applicable) (Error Resolution)

The Form 4 (Trust) is disapproved for the following reasons, only:

1. Fingerprint Card (FPC) never received. (As per Instructions on form)
2. Photo never received.
3. Physical Address is missing.
4. Serial Number is Incorrect
5. Not registered to Transferor
6. CLEO is blank, or CLEO is in different Stat from the applicant.
7. An Unlawful reason provided for reason obtaining Firearm (Section 13)
8. Discrepancies between RP's in Trust and on Form 4
9. Prohibiting question(s) answered, "Yes" or Blank/Missing Questions (Paper)
10. State Law Violation
11. No response to request for corrections (if applicable) (Error Resolution)
