What to use as the Manufacturer on ATF 5320.20 when FMI is listed

ATF eForm 1 and ATF Form 1 applications when making your own NFA firearm will show the manufacturer as FORM 1 REGISTRATION UNITED STATES "FMI" in Box 4 of the approved tax stamp.  But will you also use FMI as the manufacturer on the ATF 5320.20?  No you won't.  You will use the information that is located in 3b of your tax stamp application in that case.  

Individual Applicant Example:

If your information is listed like below, the manufacturer would be Mike Smith.

Mike Smith
123 Fake St
Boonville, MO 65203

Gun Trust Applicant Example:

If your information is listed like below, the manufacturer would be Mike Smith Gun Trust.

Mike Smith Gun Trust
123 Fake St
Boonville, MO 65203
