What Do I Need to Bring to My FFL for an ATF eForm 4?

When you are applying using the ATF eForm 4, you will need to use a FFL who has linked their FFL to their eForm account.  Below are the items that you will need to bring with you when you go to your FFL. 

Applying as an Individual*

  • Your ATF eForm Account Email Address - If you don't have an ATF eForm account you can view our walk-through guide here to create an ATF eForm account.
  • Your ATF eForm Account PIN number.
  • Completed FD-258 Fingerprint Cards or .EFT file.
  • Digital Passport Photo.
  • Form of Plastic Card Payment. 

*Items included in this list can in some cases be completed at your FFL.  You can call your FFL to verify if you can complete them at your FFL. 

Applying as a Gun Trust*

  • Your ATF eForm Account Email Address - If you don't have an ATF eForm account you can view our walk-through guide here to create an ATF eForm account.
  • Your ATF eForm Account PIN number.
  • Completed FD-258 Fingerprint Cards or .EFT file for each responsible person.
  • Digital Passport Photo for each responsible person. 
  • Completed ATF 5320.23 for each responsible person.
  • Scanned copy of your gun trust and any executed amendments.
  • Form of Plastic Card Payment.
