What is the FD-258 Fingerprint Card?

The FD-258 fingerprint cards is the official fingerprint card that is used and accepted by the the FBI, Bureau of U.S. Citizenship and Immigration Services, and many police agencies for noncriminal fingerprinting such as for employment clearances or immigration purpose. The FD-258 fingerprint card is the standard fingerprint card is generally used for: nursing licensing applications, student applications, ATF tax stamp applications, job applications, citizenship applications, etc.  The current version is version: 11-1-20.

The top portion of the FD-258 fingerprint card contains demographic fields that are relevant to the individual being fingerprinting that are completed.  The lower portion of the FD-258 fingerprint card contains an area to apply the individuals fingerprints to for both hands.  Most commonly a inkless ink pad is used to create the fingerprints on the fingerprint card. The back of the FD-258 fingerprint card contains information about the FBI, uses for the FD-258 fingerprint card, Privacy Act Statement and instructions. An example completed card can be seen here.
